OK worked it out, it's definately not on the HUDL. It's a Game on the V boxes - see the attachment. This pop up appears after you die for the first time by the looks of it.
By default the PIN is not required for game purchases on the V boxes, however this can be turned on via Settings > Parental Control Settings > Games

Wasn't there a contract at some point for this place to improve VM's reputation or something like that?
If there is a Tivo app for android, could it be used to control the tivo/pay for a game without the tv actually being on? In a headless remote control type of way if you see what I mean. Figured it must be possible, even by accident if you you can't see the tv prompts about the payment.
Glad the problem is sorted anyway.

Not impossible, but not anything you would do unless by complete random chance.
But as mentioned above, the game was on the V box, not TiVo, so no connected app for that.
