A pair of amber flashing signals 900 metres from the bridge to alert drivers it was being raised were turned off.
Two more flashing lights 200 metres from the bridge were faulty — with one off and the other emitting a steady beam.
It meant the first warning the lads got were the red lights 70 metres from the crash.
