I know that not all perks are supposed to be realistic, but the chances of anyone taken out by a headshot dropping a live grenade / pulling a trigger on a suicide bomb, when they have previously been running around with a weapon in their hand, is not going to happen. It's a perk which does nothing for the skill level of a player.

Funny enough, I hate the marty perk, but that's exactly what can happen! As your shot your muscles contract.... boom!!
Hence the reason they very rarely shoot in hostage scenarios
