It isn't that some areas don't fully support Call Waiting, it's just that Call Waiting works differently depending on the switch type at your local franchise. System X switches give the message "Please hold the line whilst we try to connect you, the person you are calling knows you are waiting." The DMS switch just gives the standard ring tone - as do all of the mobile networks I have ever used.
Some years ago BT changed their systems so that incoming callers received a ringing tone instead of a voice message, so many people complained that they changed it to what it is today - two rings alternating with the voice message.
If Call Waiting is working correctly then obviously the subscriber will receive the bleeps whilst already on an active call.
