On mine, I have one SCART from the V+ output to the TV, and another from the VCR output on the V+ to the Line 3 input on the recorder.

I re-copied the programme yesterday, without removing any cables etc., and it copied over just fine. Seems to be an intermittent problem, but I've no idea what caused/causes it.
