Call faults. When it happened to me 8 months ago it was due to cable being degraded and potentially damaged, so they pulled and relaid a new one and it was fine.

Recently I had the problem with a set of channels after the V+ was installed - only happened on the V+. Took a few weeks and a few engineers, but finally got it sorted by finding it a new point on the cabinet.

Unless they know of an area fault it's definitely worth investigating. We/they tried a few things before they were able to trace the problem each time.
