Having watched it again I'm curious as to the bit with Donna sitting on the steps hearing something.
Is it just me or did it sound like drumming?

I thought it was a heart beating, myself... It would make sense that she was upset that Earth had disappeared and she wouldn't be able to see her family again.

Yes Donna clearly hears a hearbeat and also other things that have been said to her, I feel she has something to do with the finale.
The new trailer does have Davros spouting crazyness but also there is clips of The TARDIS flying through the Crucible and then in the console room while it is exploding and Donna is looking in trouble.
Maybe she is the Rani or something. I think it will be Donna that ends up saving the day.

Thinking about it, I don't think Donna will be the one who dies - when she was sitting on the stairs, that albino woman said something about being sorry for her loss that was yet to come.

Maybe 'eternal death' gives weight to my suggestion earlier that all the Doctor's friends die but he's able to bring them all back but 1 and has to decide which one, therefore that unfortunate person has 'eternal' death. Then that could be more guilt for the Doctor to boost his lonely 'tortured soul' mentality

Donna's lose could just be her memory as I have heard.
Most loyal could be Rose or Sarah Jane, but as SA has her own series she can't really die.

Regarding the regeneration...
I very much doubt that The Doctor will regenerate into someone else - I'm sure it'll still be David Tennant.
IIRC, the only reason we've got specials next year instead of a full series was because DT needed some time off for the RSC. If he actually leaves at the end of this series, then what's the point in having specials in 2009 instead of a full series 5?
Maybe it'll be something like this
Rose - "Oh, Doctor! You haven't changed!"
Doctor - [looks at reflection] "Oh yes!..."
Rose - "What's going on?"
Doctor - "Oh, it's probably just wibbly wobbly timey wimey stuff. Anyway, enough of that - we have to stop Davros!"
Hah, & maybe when the faithful companion dies, The Doctor can cry out "Caaaaaaaaaaaaannnnnnnnnnn!"
