I suppose death, because it is final. If you fail at something, you always have another chance to make amends, if not at that thing, but something else. But death is final. There is nothing you can do after the fact.
I don;t so much as fear death per se (we all gotta go sometime) but for me I fear a horrible painful death. Like being burnt alive or something.

If I hadn't already experienced it I may have chosen this as my first (most) fear....
Which is why I chose the coffin thing, as I've not (and don't plan to I must admit ) experience it.
