Firemen and policeman (and most of all soldiers) know *at the time they choose their career* that they are expected to be sent in specifically in the worst cases humanity has to offer. They tend to be special people who are exhaustively trained to deal with this (in particular people who can handle excitement yet can stay cool under rapidly changing circumstances).
Train drivers are trained to drive trains, which requires different qualities - steadiness, high levels of concentration and, importantly, a tendency *not* to seek excitement*. These are in some ways opposed to the characteristics required for emergency service personnel.
The first thing emergency service personnel do when the brown stuff hits the rotating ventilator is let the training take over. The first thing the driver does is pick bits of person off himself, if he survives. That's a hell of a shock.
What about nurses and doctors, anyway?
*I don't fancy having a bored driver try and see if he can drive with his feet, or see if he can lift the wheels off on a bend.
