NTL Welcome Guide.
Came across the NTL Welcome Guide whilst searching for something and it has a lot of useful information in it on all NTL services, not just telephony as the URL may suggest. The Midlands edition is at http://www.ntl.com/home/telephone/pdf/wpack09.pdf

There isn't one I know of for the Republic but the Belfast area should be covered by the Central one.
